How Kestrel Health Staffing brought per diem, stipend and travel compliance to 1,380 clinicians across 41 states
Kestrel places travel nurses and allied health clinicians on 13-week assignments. Its clinicians claim mileage, lodging, licensure fees and home-office stipends from wherever they are working, and every assignment city has a different per diem rate.
The challenge
Kestrel's clinicians move every 13 weeks, often across state lines. Lodging and meal reimbursements are tied to the GSA rate for the assignment location, and the company also pays a monthly home-office stipend of $65 to recruiters and credentialing staff who work remotely from 23 states. Expensify handled submissions, Brex handled cards, and a five-person payroll operations team handled everything in between.
Errors were constant and mostly honest. Clinicians applied the rate from their previous assignment city, claimed mileage from home instead of from the tax home on file, or submitted a licensure renewal fee that Kestrel had already paid directly through Brex. Industry benchmarks put the share of expense reports with errors at around 19%, and Kestrel's internal sample came in close to that.
Because clinicians are often on night shifts, a phone call from payroll operations usually meant a two-day game of voicemail. Reimbursement cycle time had stretched to a median of 9.4 days, and slow reimbursement was showing up in exit interviews as a reason clinicians chose a competing agency for their next assignment.
How they rolled out Phyto Nex AI
Kestrel connected Expensify, Brex and QuickBooks Online and loaded each clinician's assignment city and dates from its scheduling export. Phyto Nex AI applies the GSA lodging and M&IE rate for the assignment location on the date of the expense, and recalculates automatically when an assignment is extended or moved.
Home-office stipend claims are validated against the stipend policy: one claim per calendar month, capped at $65, receipts required for any single item over $25. Co-working day passes are allowed for recruiters only when the venue is within 40 miles of their registered home address, which Phyto Nex AI checks against the merchant location on the card transaction.
The pushback agent writes to clinicians in plain language and avoids finance jargon entirely. A typical message explains the correct rate for their assignment city, shows the difference, and offers a one-tap fix. Messages are queued by local time so nobody gets pinged in the middle of a night shift. Payroll operations approves every message before it sends.
The results
In two quarters Phyto Nex AI flagged and Kestrel recovered $47,915 in over-per-diem lodging, duplicate licensure fees and mileage miscalculations, with 94.2% of clinicians resolving flags without needing a phone call.
Median reimbursement cycle time fell from 9.4 days to 3.1 days because clean reports now flow straight through and flagged reports get a precise, same-day question. Payroll operations reclaimed 17.5 hours a week, which the team redirected to onboarding paperwork for new assignments.
Kestrel's VP of Clinician Experience now tracks reimbursement speed as a retention metric. In the two quarters after rollout, reimbursement complaints in exit surveys dropped from the third most cited reason for leaving to outside the top ten.
